At Brightcreek, we strive to identify and provide the individual services and needs required in the daily care of our memory care community residents. Prior to moving in, all residents will receive a thorough assessment from our Health Services Director or other qualified assessor to determine the current level of support required. Below are a typical list of needs and services which determine the required level of care along with the associated charges for each level. All charges are in addition to the monthly base rent. In using this guide it is important to keep in mind that we will need to perform a follow-up assessment within 30 days of moving in to review resident’s actual abilities, behaviors, and requirements to be sure that the services meet those needs.
Brightcreek Amenities Included In Base Rent From $3,600
· 3 meals a day plus snacks
· Our life enhancement program
· Simple medication management (3 meds/3 times daily)
· Assistance with 2 showers weekly
· Redirection and assistance to activities
· Scheduled transportation
· Housekeeping & linen service
· Basic utilities
Level I (add the following): +$450
· Enhanced Medication Management (up to 5 meds/4 times daily), and/or
· Special Diets, and/or
· Coaching to activities
Level II (add the following): +$900
· Complex Medication Management (Diabetic Care/Inhalers), Coumadin Management, and/or
· Bowel or Bladder Incontinence
Resistive to Care (add to base rate or any level): +$500
In addition, some residents may require assistance beyond what is listed above or will need services which require additional staff assistance or extended time for any of the services above. Additional fees may be assessed if this occurs. We will provide an individual assessment and determine the actual need and service fee to ensure that the resident’s requirements are met.
